Then, you have his physical issues, which have had an effect on the first part. However, they are a concern, especially for a player who has struggled previously too.
Finally, you have his contract renewal. It is and has been on hold for some time now, and there are no signs that that will change.
The Pulisic problem

I’ve always thought he would leave as soon as Allegri was hired. He has never fit into Allegri’s system. Yes, he can play in it, but not really excel. Leao will always be the second forward in the 3-5-2 when they get a real #9. So, I think this is just softing the impact of him pushing to leave.
He is weak mentally. Everyone keeps ignoring when did he stop scoring. One fake news article and he fell apart. They should hire someone to work with him on that.
